Welcome to Prescribing Lifestyle, PLLC, a healthcare practice that is dedicated to delivering personalized and comprehensive healthcare services to its patients. The following terms of service govern your use of my services as a board-certified family and lifestyle medicine physician.

  1. Scope of Services: Prescribing Lifestyle, PLLC, provides medical services via telehealth that are focused on holistic and lifestyle-based approaches to healthcare. Board-certified lifestyle medicine and family medicine physician Dr. Polina Sayess, provides medical consultations, diagnostic tests, and treatment plans tailored to meet each patient's unique needs and goals.

  2. Medical Disclaimer: While Prescribing Lifestyle, PLLC, is committed to providing the best possible medical care, Dr. Sayess cannot guarantee specific results or outcomes. Dr. Sayess provides medical consultations, diagnostic tests, and treatment plans to support each patient's health and wellbeing.   3. Privacy Policy: Prescribing Lifestyle, PLLC, is committed to protecting the privacy and confidentiality of patients. Dr. Sayess collects and uses patient information only for the purpose of providing medical services, and maintains strict confidentiality and security standards to ensure that patient information is protected.   4. Payment and Fees: Patients are responsible for payment of all fees associated with medical services. Payment is accepted by credit card, debit card, and other payment methods. Prescribing Lifestyle, PLLC offers competitive and affordable pricing, and fees reflect the high-quality of medical care provided.

  5. Appointment Cancelation Policy: Patients who wish to cancel or reschedule a medical consultation must provide at least 24 hours' notice. Failure to do so may result in a cancelation fee of $70.

  6. Consult Time Purchased: There is no expiration date on consultation time purchased. All purchased consultation times are non-refundable and non-transferable. If memberships are canceled, the remaining consultation time can be used within the 30 day notice period, any unused minutes can be refunded at patient’s written request (minus the original discount). There is an admin fee of $70 for processing consultation time refunds.

  7. Membership Fees: Quarterly subscription fees are non-refundable. Annual subscription fees are refundable pro-rated for the remaining time. Patients who wish to cancel their membership must provide at least 30 days notice to Prescribing Lifestyle, PLLC. There is an admin fee of $70 for processing annual subscription refunds.

  8. Telehealth Services: Prescribing Lifestyle, PLLC, offers telehealth services that are compliant with the Health Insurance Portability and Accountability Act (HIPAA) standards. These services are available only to patients who are located within the states of Connecticut, New Hampshire, Maine, Massachusetts and Vermont. By using Prescribing Lifestyle, PLLC, telehealth services, you acknowledge and agree that you are physically located in either Connecticut, New Hampshire, Maine, Massachusetts or Vermont at the time of the visit.

    Please note that telehealth services are provided solely for the purpose of providing medical care to patients. Prescribing Lifestyle, PLLC, does not guarantee the availability or reliability of any telehealth services. The patient is responsible for ensuring that their internet connection is secure and reliable, providing accurate and complete information to Dr. Polina Sayess during the telehealth visit.

  9. Consultation Duration: Please be aware that the consultation duration will be based on the actual time spent during the consult. For instance, if a follow-up was scheduled for 15 minutes but in reality takes 30 minutes, then the 30 minutes would be charged, as this accurately reflects the care provided.

By using these Prescribing Lifestyle, PLLC, services, you agree to these terms of service, as well as any additional terms and conditions that may apply to specific medical services or treatment plans.
